The Opportunity:
The Director of Health Equity leads the development and implementation of a comprehensive framework to improve health equity and address social determinants of health (SDOH) barriers to improve access to quality care and health outcomes for AlohaCare members. This framework includes a comprehensive program description, workplan, and annual evaluation that is embedded within AlohaCare’s overall Quality Assurance and Performance Improvement (QAPI) program and the Population Health Management Strategy. In close collaboration with the Director of Quality and Director of Population Health, this role will oversee the strategic design, implementation, and evaluation of the Cultural Competency and Health Equity Plan aligned to AlohaCare’s Quality, Population Health, and other applicable programs including accreditation. To improve health equity and reduce social determinant of health barriers, this role will develop external and internal partnerships that support person-centered and culturally relevant healthcare for AlohaCare’s Native Hawaiian, Pacific Islanders, and other underprivileged members and their families. This position will represent AlohaCare externally as well as advance internal team member understanding of Native Hawaiian culture and health needs as well as other underprivileged populations through innovative healthcare transformation of SDOH support through member benefits in partnership with Plan Operations and Health Services.
The Director of Health Equity is responsible for working closely with AlohaCare stakeholders to create pathways and develop solutions needed to create health equity for all through a root cause focus.
The Director of Health Equity has deep expertise and competence in developing value-based community strategies, is a strategic thinker and executer with strong relationship management skills and a passion for health equity and social health.
Primary Duties and Responsibilities:
- Develop, implement, and oversee a health equity plan that integrates innovative strategies to address disparities and promote equity for AlohaCare members.
- Collaborates with Executive Council members and AlohaCare departments to integrate strategies and tactical direction for building and maintaining strong external relationships with organizations that address Native Hawaiian population health and other underserved populations, behavioral health, telehealth and social determinants of health, through community organizations, health systems, government agencies and public policy.
- Partners with Director of Quality and Director of Population Health to design and execute targeted programs aimed at improving outcomes for underserved populations and Quality HEDIS performance.
- Lead the creation and refinement of policies and procedure (P&Ps) that embed health equity and SDOH principles across the organization.
- Ensure alignment of health equity and SDOH initiatives with regulatory and accreditation requirements, industry bet practices, and organizational goals.
- Develop and manage the SDOH and CLAS programs, workplans, and evaluation ensuring they are actionable, measurable, and effective in improving health equity.
- Establish metrics and conduct regular evaluations of health equity programs to assess effectiveness and identify areas for improvement.
- Identify and address SDOH barriers that impact member health outcomes, coordinating efforts across departments and with external partners.
- Collaborate with internal departments, including Clinical Services, Quality, Population Health, Operations, Provider Relations, Behavioral Health, Finance, Data and others to integrate health equity initiatives into workflows.
- Develop and implement measures that validate the delivery of culturally competent and appropriate services to Native Hawaiian, Pacific Islanders, and other underserved populations as identified by other unique demographics in keeping with AlohaCare’s Cultural Competency Plan.
- Build and maintain partnerships with community organizations, providers, and other external stakeholders to advance health equity and SDOH goals.
- Responsible for influencing and driving adoption of health equity and SDOH programs and best practices across AlohaCare. Advocate for health equity, providing guidance and training to staff and leadership on equity-related issues.
- In conjunction with AlohaCare’s Product Management, Quality and Health Services team’s designs and develops programs to address social determinants of health (SDOH) for Native Hawaiian, Pacific Islanders, and other underserved populations.
- Informs AlohaCare member benefits and services for members and providers which address greatet equity.
- Collaborate with internal partners to develop training and program materials to educate internal and external stakeholders and contribute to member outreach and retention efforts.
- Responsible to maintain AlohaCare’s confidential information in accordance with AlohaCare policies, and state and federal laws, rules and regulations regarding confidentiality. Employees have access to AlohaCare data based on the data classification assigned to this job title.

Organizational Liaison
- Applies an understanding of health equity to non-clinical and clinical activities to effectively attain the highest level of whole health for all regardless of race, ethnicity, disability, sexual orientation, gender identity, socioeconomic status, geography, preferred language, or other factors that affect access to care and health outcomes.
- Represent AlohaCare in various settings involving organizations and agencies that address health equity facing communities we serve such as cultural and human services organizations, health systems and government agencies.
- Advance community understanding of the value of AlohaCare's model of care as a means for improving community driven health equity solutions and interventions.
- Authentically engage whole person, whole family, and whole community health to promote well-being for AlohaCare's members.
Required Qualifications
- Minimum of 5 years in a health and/or social determinant of health-related profession.
- Ability to successfully collaborate and incorporate input from diverse teams across AlohaCare to achieve success measures.
- Leadership skills to drive accountability/results across departments and among various levels of personnel.
- Strong analytical skills. Ability to problem solve, analyze data, systems map, think critically, exercise sound judgment to take action and follow-up.
- Capable organizational, project management and stakeholder management skills.
- Excellent verbal and written communication skills.
- Must be a self-starter, be able to think creatively, take responsibility, and be extremely resourceful. Ability to multi-task, adapt to changing priorities, manage innovative products in a fast-paced environment.
- Ability to work independently with strong self-motivation.
- Working knowledge of Native Hawaiian protocol, customs, traditions and culture.
- Positive working relationships with leaders of Native Hawaiian communities.
- Ability to work under minimal supervision.

- Intermediate skill using Microsoft Programs; Word, Excel, Outlook and PowerPoint.
- Possession of a valid driver’s license and auto insurance.

- Demonstrated evidence of innovative thinking in a requirement driven industry.
- Demonstrated experience and familiarity working with agencies, business, communities and individuals serving the Native Hawaiian community, Pacific Islander community, and other underprivileged communities.
Required Education
Preferred Education
- Master’s degree in healthcare related field and/or public health or relevant experience.
Mental, Physical and Environmental Demands:
- Sedentary Work: Exerting up to 20 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time.

Salary Range: $111,000 - $140,000 annually
